Defining covered individuals

Covered individuals—your employees plus the employees' beneficiaries and dependents receiving healthcare coverage through the employees' coverage—are required on each employee's 1095-C form if your company offers employer provided self-insurance coverage.

When generating ACA forms using the Forms Wizard (My Workforce Analyzer > Forms tab > Generate Forms panel), you can choose to use the dependents and beneficiaries from your Sage HR program, or import covered individual information using a file.

Using dependents and beneficiaries from your Sage program

In order to use the dependents and beneficiaries from your Sage program, you must have an HR program that you use to set up and manage your employees' dependents and beneficiaries.

Select this option in the Forms Wizard and My Workforce Analyzer retrieves the information then completes the 1095-C Employer Provided Health Insurance Offer and Coverage. You can review and edit the forms after they are generated.

Importing covered individuals

If you select to import the covered individuals, you must prepare the information in a file following specific data requirements.
